Chinese clinical practice guideline for compression therapy in peripheral vascular diseases (2026 edition)

摘要:

压力治疗是利用压力治疗器具对局部组织施加持续性或间歇性压力，以促进静脉与淋巴回流、减轻水肿、缓解症状并促进创面愈合的物理治疗方法，在静脉和淋巴系统疾病的综合治疗中具有重要作用。当前，我国压力治疗领域仍存在循证证据不足、产品标准不统一及临床应用不规范等问题，缺乏基于循证医学证据的临床实践指南。为此，中国微循环学会周围血管疾病专业委员会压力学组组织全国17家大型三甲医院相关领域专家，依据《中国制订/修订临床诊疗指南的指导原则（2022版）》及国际实践指南报告规范（RIGHT）等方法学文件，围绕8个主题领域构建32个临床问题，采用证据评价与推荐意见分级、制定和评价体系进行证据质量评价和推荐强度分级（GRADE），并通过德尔菲法形成专家共识，最终提出53条推荐意见。本指南旨在规范我国静脉与淋巴疾病的压力治疗临床实践，提高诊疗的科学性和一致性，并促进相关研究与规范化应用的发展。

Abstract:

Compression therapy is a physical treatment modality that applies continuous or intermittent pressure to specific body regions using compression devices to improve venous and lymphatic return, reduce edema, alleviate symptoms, and promote wound healing. It plays an essential role in the management of venous and lymphatic disorders. However, challenges remain in China, including insufficient high-quality evidence, lack of unified product standards, and non-standardized clinical practice, and evidence-based clinical practice guidelines are still lacking. To address these issues, the Pressure Group of the Peripheral Vascular Disease Professional Committee of the Chinese Society of Microcirculation organized experts from 17 major tertiary hospitals nationwide. Following the Guiding Principles for Developing/Revising Clinical Practice Guidelines in China (2022 Edition) and the RIGHT statement for reporting practice guidelines, the panel developed 32 clinical questions across eight key topic areas. Evidence quality and recommendation strength were assessed using the GRADE approach, and expert consensus was achieved through the Delphi method, resulting in 53 recommendations. This guideline aims to standardize compression therapy for venous and lymphatic diseases in China, improve the scientific rigor and consistency of clinical practice, and promote further research and evidence-based application in this field.
